The Uncomfortable Truth About "Last Reviewed"
Here's a question that will ruin your afternoon: when was the last time someone actually read your most-viewed knowledge base article?
Not edited it. Not fixed a typo. Not clicked "Approve" in a review queue. Actually sat down, read every sentence, and confirmed it still matches reality.
If you're like most support teams, the honest answer is: you have no idea. And your "Last Reviewed" timestamp is lying to you about it.
The Typo-Fix Loophole
Every major platform — Zendesk, ServiceNow, KnowledgeOwl — has the same fundamental flaw: any edit resets the freshness clock.
Someone fixes a comma? Article marked as "recently updated." Someone corrects a heading capitalization? Back to the top of the "recently reviewed" pile. The metric looks healthy. The content might be wildly wrong.
ServiceNow's default behavior resets the "Valid to" date to 12 months in the future on any edit. KnowledgeOwl drops articles out of audit review lists when content changes are saved. A typo fix actively removes the article from the review queue.
Think about that. The act of caring enough to fix a comma protects the article from being checked for accuracy.
It's like a smoke detector that resets every time you walk past it. Sure, the light is green. But is there a fire?
The Rubber-Stamp Problem
So you set up review cycles. Quarterly sweeps. Assigned reviewers. Checklists. You're doing it right.
Except the KCS Consortium has bad news: when review completion becomes a metric, people complete reviews without reading. Organizations report "96% of articles reviewed on schedule" while significant portions of their knowledge base are factually wrong.
The incentive structure is broken. The goal becomes finishing the review, not verifying the content. Click "Approve." Move on. The metric ticks up. The article stays wrong.

Why "Last Reviewed" Is Epistemologically Meaningless
Notre Dame librarians have known this for decades: a "last reviewed" timestamp tells you when someone interacted with an article. It says nothing about whether the content is accurate.
The date is a record of activity, not a certification of truth.
Yet every support platform treats it as proof of quality. "This article was reviewed 3 weeks ago" is supposed to mean "this article is correct." It doesn't. It means someone opened it 3 weeks ago. Maybe they read it. Maybe they skimmed the title and clicked approve. Maybe they were clearing their review queue before lunch.
A timestamp is not a verification.
What We Built Instead
When we started building cStar's audit system, we had one rule: don't repeat what everyone else is doing. Because what everyone else is doing clearly isn't working.
Verified, Not Just Edited
cStar tracks two timestamps: updatedAt (any edit) and lastVerifiedAt (explicit review). Staleness is calculated from the verification date, not the edit date.
Fix a typo? Cool. The staleness clock keeps ticking. To reset it, someone has to actually open the article in a purpose-built review flow, confirm specific things are still accurate, and submit a verification.
The typo-fix loophole is closed.
Finding Problems Is Worth More Than Rubber-Stamping
Here's where it gets interesting. In most review systems, the reward (if there is one) is the same whether you mark an article "Looks Good" or "Needs Update."

When an article owner is deactivated, their articles immediately enter the stale pool. No waiting for a quarterly review. No hoping someone notices. The system flags them, and Bob starts assigning review quests.
Articles don't fall through the cracks because the person responsible left.
Escalation That Actually Escalates
A stale article doesn't just sit there with a yellow timestamp. cStar uses a progressive escalation ladder:
Bob quest — a random agent gets assigned to review it
Owner notification — if still stale after 2 weeks, the article owner gets flagged directly
Manager escalation — 2 more weeks? The manager hears about it
Visible badge — 30+ days past threshold, the article gets a "Needs Review" badge that everyone can see
The article can't hide. The system doesn't forget. And it never auto-archives — that's always a human decision.
The Scorecard Problem (Tickets Too)
Knowledge base auditing is half the story. The other half is ticket quality.
Most platforms offer star ratings. One to five. Maybe a notes field. That's not an audit — that's a Yelp review.
cStar's ticket QA scorecard lets teams define weighted criteria: accuracy, tone, completeness, timeliness, policy adherence. Each criterion scored individually. Some marked as auto-fail — a score of 1 on accuracy fails the entire audit regardless of other scores.
